Why is 10 mg so much smaller than 100 mg sildenafil? Is it weaker?
QT first. Then the 10 mg start
The US Levitra label asks for 10 mg about 60 minutes before sex, once a day, for most men. The dose may rise to 20 mg or fall to 5 mg. Sexual stimulation is required. I still do not write that 10 mg until the QT question has a clear answer.
This is not theatre. It is the order the warning section already wrote. A small milligram does not make a QT-avoid drug into a light social tablet. Ten milligrams is the therapeutic dose that was studied for QTc, not a homeopathic gesture.
If the list is clean and the ECG story is ordinary, 10 mg is a normal start. Onset for many men sits inside 25 to 60 minutes. The useful span is about 4 to 5 hours. That is an occasion clock, like sildenafil, not a 36-hour leftover.
Vardenafil never had the launch noise of the first tablet or the weekend slogan of tadalafil. That is a marketing history. It is not a potency ranking. I file it when a short clock is wanted and the QT line is clear. I file something else when the line is not.
Partners often ask why we 'make a fuss' over a 10 mg tablet. Because 10 mg is the therapeutic start, and it is the dose in the QT study. The fuss is the list, not the milligram printed on the foil.
High-fat food slows the peak, not the ban
Levitra film-coated tablets may be taken with or without food. A high-fat meal still slows the peak. European information on a 57% fat meal puts median Tmax about 1 hour later and Cmax about 20% lower. AUC stayed similar. The evening arrives a bit late, not cancelled.
An ordinary dinner is usually quieter than that study meal. I still prefer a lighter plate on night one so we are not arguing with both a delayed peak and first-time nerves. If the plate will not move, tadalafil ignores food. That is a reason to pick a different clock, not to double vardenafil.
Orodispersible 10 mg Levitra is not bioequivalent to the film-coated 10 mg tablet. Do not swap those forms as if the number on the box made them the same start. This note is the swallowed film-coated 10 mg unless your box says otherwise.
Alcohol still drops pressure and dulls arousal. The QT line does not make a third drink safer. Keep the first week readable.
Grapefruit is a CYP3A4 story again. I would skip the juice that day. The more important bottles are ritonavir, cobicistat, ketoconazole and clarithromycin. Those change the 10 mg start into a capped dose I will not guess from memory in a Saturday letter.
Who should not start Levitra at all
Gates before a 10 mg Levitra start
- Do not start: congenital long QT; Class IA or Class III antiarrhythmics.
- Ask first: other QT-active drugs, low potassium, protease inhibitors.
- Still banned: organic nitrates and nitrites, same as tadalafil and sildenafil.
- Dose caps exist: ritonavir and other CYP3A4 inhibitors are not a 10 mg story.
Congenital long QT: do not start. Acquired long QT with a known history: same caution. Class IA antiarrhythmics - quinidine, procainamide - stay off the combination. Class III - amiodarone, sotalol - stay off as well. That is the labelled avoid list. I do not invent a careful exception around it.
Hypokalaemia is named in European product information as a risk setting where QT-prolonging drugs, including vardenafil, are best avoided. If potassium is already a problem, this is the wrong week to start 10 mg so we can 'see how the weekend goes'.
Strong CYP3A4 inhibitors raise vardenafil sharply. Ritonavir: no more than 2.5 mg in 72 hours. Several other potent inhibitors cap at 2.5 mg in 24 hours. Erythromycin and ketoconazole 200 mg daily: no more than 5 mg in 24 hours. A 10 mg start beside those bottles is the wrong start.
Nitrates close the door for the whole class, vardenafil included. If a spray is in the pocket, the QT conversation is already secondary. See the cardiologist first. Leave this foil closed.
A printed 'normal ECG' from five years ago is not a current QT line if amiodarone has been added since. Dates on traces matter. Bring the newest sheet, not the one that lived in a drawer.
Nitrates stay off after the QT check
Clearing the QT line does not clear the nitrate shelf. Organic nitrates and nitrites stay contraindicated. A man who passed the amiodarone question can still fail the spray question. I file both gates, not the interesting one only.
If chest pain starts after 10 mg, name the tablet before anyone reaches for nitrate. The vardenafil clock is shorter than tadalafil's. The ban is still a ban. There is no 'it has been four hours, the spray is fine now' sentence I will write.
Everyday effects look like the rest of the class: headache, flush, stuffy nose, indigestion. Blue tinge is less talked about than with sildenafil, and still possible. Dizziness on standing is the pressure effect. Rise slowly.
Erection past 4 hours: urgent care. Sudden vision or hearing loss: stop and get seen. Those two lines are class-wide. They do not wait on the QT paragraph.
Poppers are nitrites. They count. A club weekend plus 10 mg Levitra is the same banned stack as a GTN spray plus 10 mg. I will not dress that up as a lifestyle choice.
If both gates are already clear, the nitrate sentence still belongs in the first week talk. Men remember the QT fuss and forget the spray. I repeat both.
Eight milliseconds is a measured mean
Fifty-nine healthy men aged 45 to 60 sat in a crossover QT study. Ten milligrams of vardenafil produced a mean QTcF increase of 8 msec versus placebo (90% CI 6 to 9) at about 1 hour, near Tmax. Eighty milligrams, four times the labelled maximum, produced about 10 msec.
Moxifloxacin 400 mg, the positive control, sat in the same small band. Absolute QT did not rise the way a torsadogenic story would want. The clinical impact of that 8 msec shift is unknown across a mixed clinic list. Unknown is not the same as harmless in every man, and not the same as a panic.
A later combination study with gatifloxacin, another QT-active drug, showed an additive QTc effect of about 4 msec versus either drug alone. Additivity is why I ask for the whole list, not just 'heart tablets'.
Sildenafil 50 mg in a related design produced a smaller mean QTcF shift, about 6 msec. The avoid language on Class IA and Class III drugs is still written on vardenafil in a way I treat as a start-gate. I do not shrug it because 8 looks close to 6.
I do not order a Holter on every healthy man before 10 mg. I do ask whether anyone has ever said 'long QT', and I read the full list. That takes a minute. It prevents a stupid start.
Sixty minutes, one tablet, arousal still required
10 mg after the QT line is clear
Once the QT line is clear, 10 mg about an hour before sex is the ordinary US start. European wording allows 25 to 60 minutes. Some men feel it toward the earlier end. I still tell a first-timer not to judge it at 10 minutes.
One tablet a day is the frequency cap. A second 10 mg at midnight is a stacked dose. If 10 mg is weak after a few aroused evenings, the labelled rise is 20 mg. If side effects lead and the effect is there, 5 mg is the step down. Elderly men sometimes start at 5 mg. That is a clinic call, not a forum one.
Moderate hepatic impairment (Child-Pugh B): start 5 mg, maximum 10 mg once daily. I do not improvise around liver numbers from a mail description. Bring the labs if they exist.
Arousal is still the first signal. Vardenafil does not invent it. A quiet first night after a fight is not a QT problem and not a 20 mg indication. Give the 10 mg start a few honest evenings before we change the milligram.
Child-Pugh C liver disease is a harder stop in some labels. I will not start 10 mg from a mailbag description of 'a bit of liver'. If liver numbers exist, they come with the list. If they do not, we wait.
What a first week usually feels like
If both gates are clear, the first week is usually a short, ordinary occasion tablet. Swallow about an hour ahead. Keep the plate reasonable. Wait for arousal. Expect a possible headache. Do not expect a new personality.
Give it a few evenings. One nervous night is a poor assay. The fuller page is the vardenafil remedy. If you want the food-proof long clock instead, that is tadalafil. If you want the first-night food drill, that is the 100 mg sildenafil note.
Because of the QT footnote, a short talk with Dr. Hartley or your own GP is part of the start, not an optional extra. Bring the list. I can file 10 mg after I have read it. I cannot file it from a slogan.
A first week that feels ordinary is a success. A first week that feels dramatic usually means food, alcohol or a skipped gate. Write those down. Do not raise to 20 mg to drown the story. The 10 mg start is already the labelled dose. Extra milligrams will not clear a skipped QT line.
