100 mg after the plate is clear

Pfizer's 100 mg diamond is the top of the Viagra ladder, not the labelled start. Most men on the US PI begin at 50 mg about an hour before sex, with room to rise to 100 mg or drop to 25 mg. This cabinet card locks the 100 mg tablet because that is the strength people type when they want the late, larger night. Food still moves the clock. A high-fat plate delays the peak by about an hour and knocks Cmax down about 29%. Take 100 mg after the plate is clear if you want the labelled fasted timing. Nitrates and riociguat stay banned. Compare the shorter clock with tadalafil 10 mg as needed and vardenafil 10 mg in the three-clock journal. A first 100 mg night is unpacked in the first-night note.

Clear the plate, then swallow the 100 mg diamond

Viagra may be taken with or without food. That line is true and incomplete. A high-fat meal delayed Tmax by a mean of 60 minutes and cut Cmax by a mean of 29% in the US label. This page locks the 100 mg tablet to a cleared plate so the hour you planned is the hour you get. Leftover sauce on the plate still counts as food.

Fasted Tmax sits between 30 and 120 minutes, median 60 minutes. The PI allows the tablet anywhere from 30 minutes to 4 hours before sex. A steak-and-sauce plate does not make 100 mg 'fail'. It makes the same milligrams arrive later and a bit lower. If the night is timed to a dinner, swallow after the plates leave, not under the pudding.

Alcohol is a separate damper. The tablet does not cancel a heavy drinking night. Stimulation is still required. Sildenafil will not create an erection in a man who is asleep, frightened, or uninterested. That sentence belongs next to the food sentence because both get blamed on 'a weak 100 mg pill'.

Revatio and other 20 mg pulmonary-hypertension sildenafil products are a different indication and a different tablet. Do not raid them to assemble a 100 mg ED night. Do not stack Viagra on another PDE5 inhibitor. Once per day is the frequency cap.

Eighty-two percent improved on 100 mg in the fixed-dose global question

In four randomized fixed-dose studies (1797 men, 12 to 24 weeks), 82% of men on 100 mg said erections improved, against 74% on 50 mg, 63% on 25 mg, and 24% on placebo. Median baseline on the two principal IIEF questions was 2, 'a few times'. That 82% figure is why 100 mg has a reputation. It is also why side-effect rates climb at the same step.

A diabetes trial (n=268) started men at 50 mg with titration to 100 or 25 mg. All were on 50 or 100 mg by the end. Improved erections: 57% on sildenafil versus 10% on placebo. Diary success: 48% of attempts versus 12%. Organic, psychogenic, and mixed ED all appeared in the broader program (about 58%, 17%, and 24% in the fixed-dose pool).

Titration studies (n=644), where most men ended on 100 mg, tracked the same direction. Baseline IIEF scores in untreated ED were low across firmness, maintenance, and satisfaction. Sildenafil moved those domains. Desire scores can rise a little when sex starts working. The tablet still does not replace arousal.

Those percentages do not mean 100 mg is mandatory. They mean 100 mg was the top of a dose-response the label already shows. A man who is dizzy or blue-tinged on 100 mg belongs on 50 or 25, not on a second 100 mg 'to push through'. Failures on a full stomach also do not justify a double tablet.

Fifty milligrams is the usual start; 100 mg is the labelled ceiling

For most patients the recommended Viagra dose is 50 mg as needed about 1 hour before sexual activity. Effectiveness and toleration can raise that to a maximum of 100 mg or lower it to 25 mg. Maximum frequency is once per day. This card still talks in 100 mg because that is the strength men ask me to defend.

Men over 65, men with hepatic impairment including cirrhosis, and men with CrCl under 30 mL/min should be considered for a 25 mg start because exposure runs higher. Alpha-blocker users also start at 25 mg. Those cuts exist so a jump to 100 mg is a decision, not a default print from a coupon site.

Ritonavir is the hard frequency cut: do not exceed 25 mg in 48 hours. Erythromycin, ketoconazole, itraconazole, and saquinavir push toward a 25 mg start as well. If those drugs are on the list, 100 mg is the wrong first tablet even if a friend 'does fine on 100'.

Pulmonary arterial hypertension dosing lives on a different label. ED sildenafil is the 25, 50, and 100 mg film-coated diamonds. Mixing the two products is a counselling failure. If 100 mg still does nothing after several aroused attempts on a clear plate, the problem is often not milligrams. Look at nitrates, alcohol, and whether desire was actually present.

Headache, flush, dyspepsia, and the 11% color-tinge at 100 mg

Fixed-dose Phase II/III tables show the 100 mg step clearly. Headache 28% (placebo 7%), flushing 18% (2%), dyspepsia 17% (2%), abnormal vision 11% (1%), nasal congestion 9%. Those are the numbers I read back when someone wants 100 mg because a shop listed it first.

Flexible-dose trials, which resemble real titration, reported headache 16% versus 4% placebo, flushing 10% versus 1%, dyspepsia 7% versus 2%, nasal congestion 4% versus 2%, abnormal vision 3% versus 0%, dizziness 2% versus 1%, and rash 2% versus 1%. Back pain was 2% on both arms. Discontinuation was 2.5% on drug versus 2.3% on placebo.

The color tinge and light sensitivity come from PDE6 in the retina, an enzyme sildenafil hits harder than tadalafil. It is usually short-lived. Sudden vision loss in one or both eyes is a different story: stop the class and seek care for possible NAION. A crowded disc, age over 50, diabetes, hypertension, coronary disease, lipids, and smoking are the usual NAION baggage. Observational work suggested about a two-fold NAION risk when a PDE5 inhibitor was used in the prior five half-lives. Causality is not proven. Caution still applies after a prior NAION.

Erection longer than 4 hours needs emergency care. Priapism past 6 hours can scar the tissue. Sudden hearing drop needs prompt care too. None of those wait for a Monday GP slot.

How a first 100 mg night gets written from Lyon

Mail first, bottle second. A 100 mg script from this practice names 'after food has cleared', once per day, and the nitrate ban in the same paragraph. Quantity is a small count, not a trophy bottle. Review after a few attempts beats a silent year of unused diamonds.

I ask what he ate the nights it 'did nothing'. I ask whether anyone still keeps GTN. I ask whether ritonavir, ketoconazole, or an alpha-blocker is on the list. Only then do I defend 100 mg instead of 50. The first-night note is for the couple who want the clock in plain language.

Color tinge on 100 mg is common enough (11% in fixed-dose work) that I mention it before the night, not after a frightened message. Headache at 28% gets the same warning. Neither is a reason to take a second diamond. Both are reasons to try 50 mg if the night was otherwise a success.

If he needs a leftover window into the next day, 100 mg sildenafil is the wrong molecule. Point him at tadalafil 10 mg as needed and at the journal clocks. If a long-QT story is the issue, vardenafil may be the one to avoid rather than the one to try.

Eight 100 mg tablets at grocery windows, not the ten-count coupon

Grocery windows in four US regions will still fill eight 100 mg tablets if a clinician wrote that count. Hy-Vee, ShopRite, Giant Food, and Hannaford are the chains on this card. The nearest published GoodRx line is 100 mg times 10. Ask them to price eight. Prescription required. Jovin does not run a till.

Brand Viagra cash prices still shock people who only saw a generic telehealth ad. Hims-style subscription copy is not a pharmacy quote for eight tablets at Hy-Vee. A real US window will want a prescription. Sites that skip that step are not where I send anyone.

Revatio 20 mg and compounded liquids are different products. Do not let a clerk 'make up' 100 mg from PAH stock. If the script says eight and the coupon says ten, reprice. ZIP moves the number. This site does not sell the bottle.

Walgreens virtual care and similar US paths still require a clinician to approve ED sildenafil. That is the legal route. A 25 mg or 50 mg fill is a different NDC from this 100 mg lock. Ask the grocery window to read the milligram stamp before they bag eight tablets. Jovin does not dispense. Insurance often refuses the ED claim even when the generic is cheap with a coupon. Bring the script, not a screenshot of a thirty-count board.

Four hours of useful effect, not a 36-hour leftover

RigiScan work in the Viagra PI showed an effect out to 4 hours, weaker than at 2 hours. That is the clinical clock I quote for 100 mg, not internet claims of a 12-hour hangover. Plasma at 24 hours after 100 mg in healthy men is about 2 ng/mL against a peak near 440 ng/mL.

Absorption
Fasted Tmax 30-120 min (median 60). High-fat meal: Tmax +60 min, Cmax -29%. Bioavailability ~41%.
Distribution
Vss 105 L. ~96% protein bound.
Metabolism
CYP3A4 major, CYP2C9 minor. Active N-desmethyl metabolite ~40% of parent levels.
Excretion
T½ ~4 h for parent and metabolite. Once-daily frequency cap.

Both parent and the N-desmethyl metabolite have terminal half-lives of about 4 hours. The metabolite sits at about 40% of parent levels and accounts for about 20% of the pharmacologic effect, with in-vitro PDE5 potency around half of sildenafil. That is why the useful window is measured in hours, not in the next calendar day.

Volume of distribution at steady state is 105 L. Protein binding is about 96% for parent and metabolite, independent of concentration. Mean absolute bioavailability is 41% (range 25-63%). Dose proportionality holds across 25, 50, and 100 mg. CYP3A4 is the major clearance route, CYP2C9 the minor one.

Compared with tadalafil 10 mg, you trade leftover Saturday for a tighter Friday. Compared with vardenafil 10 mg, you trade a QT-specific caution for a stronger food effect. None of the three is a desire drug. Pick the clock you can live with, then keep nitrates off the list.

Nitrates stay banned; 24-hour plasma is low but not declared safe

Sildenafil is contraindicated with nitric-oxide donors: organic nitrates, organic nitrites, regular or intermittent, any form. It is also contraindicated with guanylate-cyclase stimulators such as riociguat. The PI states it is unknown when a nitrate can be safely given after Viagra. Hypersensitivity to sildenafil or any tablet component is listed next to that ban.

After 100 mg in healthy men, 24-hour plasma is about 2 ng/mL versus a peak near 440 ng/mL. In men over 65, in hepatic impairment, in CrCl under 30 mL/min, and with erythromycin or strong CYP3A4 inhibitors, that 24-hour leftover can be 3 to 8 times higher. So 'it was yesterday' is not a green light in those groups.

Alpha-blockers and other antihypertensives can add hypotension. Start sildenafil at 25 mg in men already stable on an alpha-blocker. Sexual activity itself can be unsafe after a recent infarction, with unstable angina, or with uncontrolled arrhythmia or pressure. The tablet does not clear a man for sex that cardiology would forbid.

Other ED drugs in the same enzyme family are not extras to stack on 100 mg. Recreational nitrites are nitrates by another name. If chest pain starts after the diamond, emergency care and a full medication history beat any leftover spray in a coat pocket. Known hypersensitivity to sildenafil or a tablet excipient is a separate contraindication on the same PI list.

Ritonavir multiplies exposure; 100 mg is then the wrong tablet

Ritonavir, a strong CYP3A4 inhibitor, raised sildenafil AUC 11-fold. The labelled cap is a single 25 mg dose in a 48-hour period. A man on ritonavir who orders 100 mg generic Viagra from a shop has the wrong strength in the envelope. Pharmacokinetics stay dose-proportional across 25, 50, and 100 mg, so a four-fold tablet is a four-fold exposure when clearance is already blocked.

When 100 mg is not the first tablet

  • Ritonavir: maximum 25 mg once in 48 hours.
  • Erythromycin, ketoconazole, itraconazole, saquinavir: consider 25 mg start.
  • Age ≥65, hepatic impairment, CrCl <30 mL/min: consider 25 mg start.
  • Stable alpha-blocker: start 25 mg.
  • Nitrates, nitrites, riociguat: contraindicated.

Erythromycin, a moderate inhibitor, raised sildenafil Cmax 160% and AUC 182%. Saquinavir raised Cmax 140% and AUC 210%. Ketoconazole and itraconazole would be expected to do at least as much as saquinavir. Those are 25 mg-start conversations, not 100 mg-first conversations.

Healthy volunteers 65 and older had about 84% higher sildenafil AUC and 107% higher metabolite AUC than 18- to 45-year-olds. Free (unbound) AUC rose 45% for parent and 57% for metabolite after accounting for protein-binding differences. That is the age-related 25 mg consideration, not a ban on later 100 mg if someone tolerates the climb.

Severe renal impairment (CrCl under 30 mL/min) roughly doubled AUC and Cmax. Mild and moderate renal impairment did not need a cut. Child-Pugh A and B raised AUC 85% and Cmax 47%. Child-Pugh C was not studied; 25 mg is still the labelled consideration for any hepatic impairment. Grapefruit is the informal CYP3A4 extra I mention in the same letter.

Keep 100 mg tied to an empty plate and an empty nitrate list

File the 100 mg card with three restraints. Nitrates and riociguat are contraindicated, and the PI will not name a safe hour for a later nitrate. Frequency is once per day. A high-fat meal delays the peak by about an hour and cuts Cmax about 29%, so the plate has to be part of the plan. Once-daily frequency still holds after a late dinner.

If 100 mg on a clear plate still fails after real arousal, stop chasing milligrams from a second tablet. Recheck alcohol, desire, vascular disease, and whether 50 mg was ever fairly tried. If the only complaint is the short clock, change molecule rather than stacking diamonds.

Do not alter this dose from a chat thread. Take the plate timing, the CYP3A4 list, and the nitrate question to the person who will sign. Educational pages here do not replace that visit.

Revatio 20 mg remains a pulmonary-hypertension product. Do not assemble 100 mg from those tablets. RigiScan work showed a weaker effect at 4 hours than at 2 hours, so a late second attempt the same night is not a reason for a second diamond. If the 4-hour clock is the complaint, change molecule. If color tinge at 11% on 100 mg is the complaint, drop to 50 mg before you abandon the class. Hypersensitivity to the tablet stops the class as well.

Most 100 mg Viagra letters in this mailbag are really food-and-timing letters. The milligram is what people type. The plate is what usually wrecked the first night. These are typical replies.

We had a heavy steak dinner. When should he take the 100 mg tablet?

After the plates are cleared, not under the last of the sauce. The Viagra label allows food, but a high-fat meal delayed Tmax by a mean of 60 minutes and reduced Cmax by a mean of 29%. If you swallow 100 mg in the middle of that dinner and expect a 60-minute peak, you have planned the wrong hour. Fasted median Tmax is 60 minutes, range 30 to 120.

The PI still lets you take the tablet 30 minutes to 4 hours before sex, so a delayed peak is not always a ruined night. It is a shifted night. Alcohol on top of the steak is a second problem the tablet will not fix. If Friday dinners are always heavy, either eat earlier or pick a molecule that the label says may be taken without regard to food, such as tadalafil 10 mg. Do not take a second 100 mg at midnight because the first one met a full stomach. Once per day stays the cap. For a first-night walk-through, use the first-timers note.

The label starts at 50 mg. Why is this page pushing 100 mg?

It is not pushing it as a first tablet. The US PI starts most men at 50 mg and allows a rise to 100 mg or a drop to 25 mg. This page locks 100 mg because that is the strength shops advertise and the strength men ask me to justify after a failed 50 mg night they took with lasagne. Fixed-dose trials did show a step-up: 63%, 74%, and 82% reported improved erections on 25, 50, and 100 mg, versus 24% on placebo.

Side effects step up too. Headache 28%, flushing 18%, dyspepsia 17%, abnormal vision 11% on 100 mg in those fixed-dose tables. If 50 mg on a clear plate already worked, I leave it. If 50 mg failed after real arousal, no heavy meal, and no nitrate, then 100 mg is the labelled ceiling, not a second 50 mg two hours later. Men over 65, men with liver disease, CrCl under 30, alpha-blockers, or ritonavir do not skip straight to 100 mg. They start at 25 mg, and ritonavir never exceeds 25 mg in 48 hours. Send the list before you order the diamond because a shop printed 100.

He saw a blue tinge after 100 mg. Is that expected or an emergency?

A short color tinge, extra light sensitivity, or mild blur is expected often enough at 100 mg that I warn before the night. Abnormal vision sat at 11% on 100 mg versus 1% on placebo in the fixed-dose table. It is usually mild and transient, and it tracks PDE6 in the retina. Only one patient in the flexible-dose vision footnote discontinued for it. It is annoying. It is not the same as going blind in one eye.

Sudden vision loss in one or both eyes is the emergency. Stop sildenafil and get urgent care. That picture can be NAION. Prior NAION, a crowded disc, age over 50, diabetes, hypertension, coronary disease, high lipids, and smoking raise concern. Observational studies suggested about a two-fold NAION signal when a PDE5 inhibitor was used in the previous five half-lives. That is not proof of cause. It is enough for me to be blunt. Hearing drop is also urgent. An erection past 4 hours is urgent. A blue wash that fades as the 4-hour clock runs down is the counselling I give in advance so 100 mg does not produce a panic message at 1 a.m. If the tinge is intolerable, 50 mg is the labelled step down.

He is on ritonavir. Can he still use the 100 mg generic?

Not at 100 mg, and not on the usual nightly schedule. Ritonavir raised sildenafil AUC 11-fold. The Viagra label caps that combination at a single 25 mg dose in a 48-hour period. A 100 mg tablet in that setting is an overdose by exposure even if the stamp on the diamond says the same number your friend takes. I will not countersign a 100 mg generic while ritonavir is active.

Other CYP3A4 inhibitors are milder but still move the start. Erythromycin: Cmax up 160%, AUC up 182%. Saquinavir: Cmax up 140%, AUC up 210%. Ketoconazole and itraconazole should be treated as at least as strong as saquinavir. Those men consider 25 mg first. Age over 65 raised AUC about 84%. Severe renal impairment roughly doubled exposure. Child-Pugh A/B raised AUC 85%. All of those are 25 mg conversations. If the HIV regimen later drops the booster, we can revisit 50 mg, then 100 mg, on a clear plate, still once daily, still with nitrates banned. Until then the 100 mg bottle is the wrong strength to have in the house.

How long after 100 mg is a nitrate still unsafe if he gets chest pain?

The label will not give you a safe hour. Sildenafil is contraindicated with organic nitrates and nitrites in any form, regular or as needed, and with riociguat. After 100 mg in healthy men, plasma at 24 hours is about 2 ng/mL versus a peak near 440 ng/mL. The PI still says it is unknown whether a nitrate can be safely co-administered at that 24-hour point.

In older men, in liver disease, in CrCl under 30, and with erythromycin or strong CYP3A4 inhibitors, leftover sildenafil at 24 hours can be 3 to 8 times higher than in healthy volunteers. So 'it was last night' is a weaker argument in those groups. Chest pain after 100 mg is emergency care. Say a PDE5 inhibitor was taken. Do not spray GTN from a coat pocket. If he still needs a nitrate as a standing plan, he should not have 100 mg sildenafil at all. We pick another route for the ED conversation after cardiology has rewritten the angina plan. That is the same hard ban I write on tadalafil and vardenafil. Only the leftover hours differ, and even here the leftover is not declared safe. If he still carries GTN, I will not write 100 mg at all. Empty the nitrate plan with cardiology first, then we talk about a clear plate and a 50 mg start, not a 100 mg shop default.
