My ECG was called a long QT. Is 10 mg vardenafil still the start?
Read the ECG and the antiarrhythmic list before the 10 mg start
Vardenafil's difference in this cabinet is not a magic 10 mg. It is a small, dose-related QT stretch that the other two ED PDE5 tablets do not carry as a named avoid-list. Congenital long QT, class IA antiarrhythmics, and class III antiarrhythmics: do not use vardenafil. That filter comes before the labelled start. A 10 mg tablet does not get a free pass because the QTc change looked small in healthy volunteers.
Class IA examples are quinidine and procainamide. Class III examples are amiodarone and sotalol. Those names on a list stop vardenafil in this practice even if the man feels well. A family history of long QT belongs in the same pile. Other QT-prolonging drugs add a clinical judgment the PI asks you to make, including the gatifloxacin add-on study.
Nitrates and nitric-oxide donors remain a hard contraindication, as with the rest of the class. Riociguat sits in that ban. Clearing the QT line does not clear a GTN spray. Both checks happen before I write 10 mg. Sexual activity still has to be reasonable from a cardiac point of view.
Brand Levitra left US shelves. Generic 10 mg film-coated tablets did not. Orange 10 mg tablets debossed on one side are one generic presentation. Strength stamps differ by maker. Read the milligram, not the memory of a Levitra sample from 2008.
What a 10 mg Levitra letter from this mailbag actually contains
Mail the QT pass, then the milligram. A 10 mg script from Lyon names film-coated versus ODT, the once-daily cap, and the nitrate ban. I also name the 5 mg geriatric and alpha-blocker starts so a coupon page cannot overwrite them. Headache at 15% and flushing at 11% get a sentence in the same letter so 10 mg does not arrive as a surprise.
If he is 68 and otherwise simple, I often start 5 mg even though 10 mg is the usual adult label. If he is 52 with a normal QT story and no booster, 10 mg is the start I write. If he wants Saturday leftover, I send him to tadalafil 10 mg instead of inventing a vardenafil hangover.
ODT requests get a separate sentence so a substitute does not happen at the window. Alpha-blockers get 5 mg film-coated, not Staxyn, as the first PDE5 step. CYP3A4 boosters get the 2.5 mg caps, or a pause. Nitrates still empty the plan entirely.
Side-by-side clocks live in the journal piece. This card stays on the QT filter, the 10 mg start, and the ODT trap. Quantity is four tablets until we know 10 mg is tolerable. Child-Pugh B keeps a 10 mg ceiling even if the coupon page prints 20 mg.
Ritonavir can multiply AUC 49-fold; 10 mg then disappears as a plan
Ritonavir 600 mg twice daily with vardenafil 5 mg raised vardenafil AUC 49-fold and Cmax 13-fold, and stretched half-life to 26 hours. The labelled cap is a single 2.5 mg tablet in a 72-hour window. A standing 10 mg start cannot survive that interaction.
Dose caps that replace a casual 10 mg start
- Ritonavir: 2.5 mg once in 72 hours.
- Indinavir, saquinavir, atazanavir, ketoconazole 400 mg, itraconazole 400 mg, clarithromycin: 2.5 mg once in 24 hours.
- Ketoconazole 200 mg, itraconazole 200 mg, erythromycin: 5 mg once in 24 hours.
- Congenital long QT or class IA/III drugs: do not use.
- Nitrates and riociguat: contraindicated.
Indinavir 800 mg three times daily with vardenafil 10 mg: AUC up 16-fold, Cmax up 7-fold, half-life doubled. Cap: 2.5 mg in 24 hours. Ketoconazole or itraconazole 400 mg daily, saquinavir, atazanavir, and clarithromycin share that 2.5 mg per 24-hour cap. Ketoconazole or itraconazole 200 mg daily, and erythromycin, cap at 5 mg in 24 hours.
Grapefruit was not formally mapped and would likely raise exposure. Cimetidine 400 mg twice daily did not change 20 mg vardenafil bioavailability. Nifedipine SR 30 or 60 mg daily did not change vardenafil levels; 20 mg vardenafil added mean extra supine drops of 6/5 mmHg in those hypertensive men. Aspirin 81 mg times two did not get a longer bleeding time from 10 or 20 mg vardenafil.
Ritonavir also cut its own Cmax and AUC about 20% when 5 mg vardenafil was added. Indinavir Cmax and AUC fell 40% and 30% with 10 mg vardenafil. Those reverse effects do not license a casual 10 mg add-on. The vardenafil cap is still the one I write in the letter.
Four to five hours of half-life, 15% bioavailability, a modest food hit
Vardenafil is absorbed fast, with absolute bioavailability only about 15%. After a 20 mg fasted dose, Cmax usually lands between 30 minutes and 2 hours, median 60 minutes. Terminal half-life of parent and primary metabolite M1 is about 4-5 hours.
Steady-state volume of distribution is 208 L. Protein binding is about 95% for parent and M1, reversible and independent of concentration. A 20 mg dose put a mean 0.00018% of the dose in semen at 1.5 hours. CYP3A4 does most of the metabolism, with CYP3A5 and CYP2C help. M1 from piperazine desethylation is about 26% of parent levels and about 7% of total activity.
Feces carry about 91-95% of an oral dose as metabolites. Urine carries about 2-6%. Total body clearance is 56 L/h. Pediatrics are not an indication. Compared with sildenafil 100 mg, the clock length is similar and the food penalty is a Cmax cut without the same labelled 60-minute Tmax delay. Compared with tadalafil 10 mg, there is no 36-hour leftover.
Useful effect is still measured in hours after a 10 mg start, not in the next morning. Do not take a second 10 mg at 2 a.m. because the first met a heavy meal. Once per day remains the cap. If leftover Saturday is the goal, this is the wrong PDE5 tablet.
Eight milliseconds at 10 mg is small and still a named avoid-list
A crossover QT study in 59 healthy men aged 45-60 compared vardenafil 10 mg, vardenafil 80 mg, moxifloxacin 400 mg, and placebo. ECGs were read at 1 hour, near peak. Therapeutic 10 mg and supratherapeutic 80 mg produced QTc increases similar to moxifloxacin. European labelling cites mean QTc prolongations of 8 msec at 10 mg and 10 msec at 80 mg.

Heart rate rose a mean 5 beats/minute on 10 mg versus placebo and 6 beats/minute on 80 mg. Moxifloxacin 400 mg was the active QT control. A later study in 44 volunteers found 10 mg vardenafil plus gatifloxacin 400 mg additive on QTc compared with either drug alone. The clinical meaning of those millisecond shifts is unknown, which is why the avoid-list exists instead of a shrug.
I do not order a full electrophysiology lab for every 10 mg request. I do want the ECG story, the family QT story, and the antiarrhythmic list. Men already on sotalol or amiodarone get sildenafil or tadalafil in this mailbag, not a 'small QT' experiment. Hypokalaemia is another risk the European SmPC flags next to congenital QT.
Supratherapeutic 80 mg was four times the highest recommended dose. It was chosen because those levels cover a 5 mg tablet taken with ritonavir 600 mg twice daily. That is also why the ritonavir cap is brutal: 2.5 mg not more than once in 72 hours. QT and CYP3A4 are the same conversation when a booster is on the list.
Four 10 mg tablets at grocery pharmacies, not a thirty-count board

Staxyn 10 mg ODT is not a silent swap for film-coated 10 mg
Staxyn is vardenafil 10 mg that dissolves on the tongue without water. Absorption is not identical to the swallowed film-coated 10 mg tablet. US labelling and the consumer monographs treat them as non-interchangeable. Use the form that was prescribed.
Men who cannot swallow a tablet sometimes want ODT for that reason alone. That can be fair if the prescriber chose Staxyn. It is not fair if a shop substitutes ODT because film-coated 10 mg was short. Alpha-blocker patients should not start therapy on the ODT. The Drugs.com Levitra monograph repeats the same non-substitution warning the US labels carry. Mayo's oral-route monograph lists the disintegrating 10 mg tablet as its own form with its own adult directions.
Film-coated 10 mg can be taken with or without food, but high-fat meals cut Cmax 18-50% in two food-effect studies. If the night is built around a rich dinner, either eat earlier or accept a lower peak. That food story is milder than sildenafil's mean 29% Cmax cut plus a 60-minute Tmax delay, and it is still real.
Brand Levitra discontinuation in the US confuses mail-order lists. Generic film-coated 10 mg and generic ODT, where marketed, are the remaining legal paths. Import 'Levitra' of unknown origin is not a QT-screened plan. Quantity on this card is four 10 mg tablets, not a thirty-count curiosity.
Ten milligrams is the labelled usual start, not a cautious afterthought
For most patients the recommended vardenafil start is 10 mg by mouth about 60 minutes before sexual activity. The dose may rise to 20 mg or fall to 5 mg. Maximum frequency is once per day. Film-coated tablets may be taken with or without food. High-fat meals still cut Cmax.
Levitra milligrams after the QT pass
Men 65 and older should be considered for a 5 mg start because mean Cmax and AUC were 34% and 52% higher than in 18- to 45-year-olds. Phase 3 still enrolled more than 834 older men without a unique safety split at 5, 10, or 20 mg. The extra exposure is still why 5 mg is the labelled geriatric thought.
Stable alpha-blocker therapy: start vardenafil at 5 mg (2.5 mg with certain CYP3A4 inhibitors). Do not launch an orodispersible 10 mg tablet as the first PDE5 dose on an alpha-blocker. Separate the alpha-blocker and vardenafil peaks when both must run. Terazosin plus simultaneous vardenafil produced outlier standing pressures in the label work.
Child-Pugh B starts at 5 mg and must not exceed 10 mg. Child-Pugh C: do not use. Mild Child-Pugh A needs no cut. Renal impairment down to CrCl under 30 mL/min raised AUC only 20-30%; dialysis patients were not studied. Stimulation is still required. A 10 mg tablet on the tongue of a sleeping man does nothing useful.
North American 10 mg moved EF domain from 13 to 21 against placebo 15
Four major double-blind trials randomised 2431 men (mean age 57) to 5, 10, or 20 mg. Mean baseline EF domain was 11.8. In the North American fixed-dose trial (n=762), 10 mg raised mean EF domain to 21 at 3 months from a baseline of 13, versus placebo 15 from 14 (p < 0.0001). Five milligrams reached 18. Twenty milligrams reached 21.
SEP2 (penetration) on 5, 10, and 20 mg was 65%, 75%, and 80% versus 52% placebo at 3 months (p < 0.0001). The European trial confirmed the pattern. Improvement held at 6 months in North America. Special-population trials in diabetes and after prostatectomy were part of the same four-trial set.
Adverse reactions on labelled flexible and fixed doses included headache 15% versus 4% placebo, flushing 11% versus 1%, rhinitis 9% versus 3%, and dyspepsia 4% versus 1%. Discontinuation for adverse events was 3.4% versus 1.1% placebo. Dose trends showed up for headache, flushing, dyspepsia, nausea, and rhinitis across 5, 10, and 20 mg. Back pain was 2.0% versus 1.7% placebo.
Priapism, sudden vision loss, and sudden hearing loss get the same emergency speech as the other two PDE5 tablets. Color tinge is less of a vardenafil headline than a sildenafil 100 mg headline. The QT avoid-list is the headline this molecule does own. Read the start note before a first 10 mg night.
Keep the QT pass stapled to the 10 mg start
File three restraints with generic Levitra 10 mg. Congenital long QT and class IA or III antiarrhythmics: do not use. Nitrates and riociguat: contraindicated. Staxyn 10 mg ODT is not a silent swap for the swallowed 10 mg tablet. The usual start remains 10 mg about an hour before sex, once a day. Stimulation is still required after that QT pass.
If 10 mg fails after real arousal, the labelled rise is 20 mg, not a second 10 mg the same night. If 10 mg is too much flush or headache, 5 mg is the labelled drop. If Saturday leftover is the actual wish, change to tadalafil rather than stretching vardenafil folklore.
Do not start this tablet from a forum while sotalol or a nitrate is still on the list. Take the ECG story and the full medicines list to the clinician who will sign. These pages teach. They do not fill.
Absolute bioavailability is only about 15%, so CYP3A4 blockers hit this molecule hard. That is why ritonavir turns 5 mg into a 49-fold AUC problem. Keep the 2.5 mg caps on the same page as the 10 mg start. Brand Levitra leaving US shelves does not retire the QT avoid-list on generic 10 mg. Child-Pugh C still does not get this tablet. Dialysis patients were not in the pharmacokinetic studies, so I do not improvise a 10 mg start there.
