About 0.5% of men on as-needed Cialis stopped because of back pain or myalgia. That is a real stop reason, not a rumour.

Color-tinge vision is less of a tadalafil story than a sildenafil one, because this molecule barely touches PDE6 in the retina. Priapism over 4 hours, sudden vision loss, and sudden hearing loss still need emergency care, as they do with every PDE5 inhibitor. CrCl cuts, the 10 mg hepatic cap, and who should not use daily tablets

Single 5 to 10 mg doses doubled tadalafil AUC when creatinine clearance sat between 30 and 80 mL/min. On hemodialysis, Cmax roughly doubled and AUC rose 2.7- to 4.8-fold after 10 or 20 mg. Those exposure jumps, not a bedside guess, drive the dose cuts.

As-needed use with CrCl 30 to 50 mL/min starts at 5 mg once daily and should not exceed 10 mg once every 48 hours. CrCl under 30 mL/min, or dialysis, limits as-needed tadalafil to 5 mg not more than once every 72 hours. Hemodialysis between 24 and 30 hours after a dose barely moved parent drug or metabolite.

Daily tadalafil is not recommended when CrCl is under 30 mL/min or on hemodialysis. With CrCl 30 to 50 mL/min, daily use starts at 2.5 mg and may rise to 5 mg if the smaller dose is tolerated. A 10 mg clinical-pharmacology run in men with CrCl 30 to 50 mL/min found back pain as a limiting event. The 5 mg daily picture was closer to the general population.

Mild or moderate hepatic impairment (Child-Pugh A or B) caps as-needed tadalafil at 10 mg once daily. Daily use has not been well studied there, so caution is labelled. Severe hepatic impairment (Child-Pugh C) has too little data; the label does not recommend tadalafil. Pulmonary arterial hypertension uses a separate 40 mg brand (Adcirca). Do not borrow that tablet to stretch an ED night.

Healthy men 65 and older showed 25% higher AUC with no change in Cmax compared with men 19 to 45. Age alone does not force a dose cut. Greater sensitivity in some older men still belongs in the first visit. Diabetes after 10 mg lowered AUC by about 19% and Cmax by 5%. No diabetes-specific cut is labelled.

Rifampin 600 mg daily cut a 10 mg tadalafil AUC by 88% and Cmax by 46%. Carbamazepine, phenytoin, and phenobarbital would likely do something similar. The label does not raise the tadalafil dose to chase that loss. Daily-use efficacy can simply fall off. Grapefruit juice was not formally studied and would likely raise exposure.

Five or more alcohol units with tadalafil can add orthostatic heart-rate rise, a drop in standing pressure, dizziness, and headache. Plasma levels of alcohol and tadalafil did not change each other in the interaction work. Ordinary antihypertensives are usually workable if pressure is already controlled. Nitrates remain a hard ban on top of every other list.
